But what happens when the pictures don\u2019t match the symptoms? That\u2019s exactly the case with a 65-year-old endurance horseback rider who came to PT with a frustrating problem: numbness in his feet that appeared every time he spent hours in the saddle. <\/span><\/div>\n<div><span>This case highlights the power of a thorough clinical assessment and conservative treatment, reminding us that while imaging can reveal a lot, it doesn\u2019t always tell the whole story. <\/span><\/div>\n<h2><strong>The Challenge: Foot Numbness in the Saddle <\/strong><\/h2>\n<div><span>Our patient was a long-distance horseback rider who routinely completed endurance rides of more than 100 miles. Over time, he developed progressive numbness in his third and fourth toes, first on the left, then on the right. <\/span><\/div>\n<div><span>Naturally, his providers suspected his spine. After all, his imaging showed a typical list of findings: <\/span><\/div>\n<ul>\n<li><span>Chronic fractures (pars defects) at L4 <\/span><\/li>\n<li><span>A slip at L4\u2013L5 (anterolisthesis) <\/span><\/li>\n<li><span>Narrowing of the spinal canal <\/span><\/li>\n<\/ul>\n<div><span>With his imaging, it was recommended that he undergo an L4\/5\/S1 lumbar fusion. <\/span><\/div>\n<h2><strong>Looking Closer: The Clinical Exam <\/strong><\/h2>\n<div><span>Despite the imaging (which we know can be present in asymptomatic patients), his exam told a different story: <\/span><\/div>\n<ul>\n<li><span>His strength, reflexes, and sensation all tested normal. <\/span><\/li>\n<li><span>Spine-based symptoms were negative. <\/span><\/li>\n<li><span>Palpation near the\u202f<\/span><span>sural nerve<\/span><span>\u202falong the lateral aspect of his B calves reproduced his numbness. <\/span><\/li>\n<li><span>Straight leg test with sural N bias revealed symptoms on the left side more than the right. <\/span><\/li>\n<\/ul>\n<div><span>On testing, this didn\u2019t present as a classic lumbar radiculopathy. The problem wasn\u2019t coming from the spine. Instead, the sural nerve, which runs down the back of the calf into the foot, was the culprit due to overuse and prolonged standing in dorsiflexion (with knee flexion, so the soleus muscle was the primary issue). <\/span><\/div>\n<h2><strong>The Plan: Conservative Care First <\/strong><\/h2>\n<div><span>Rather than lumbar surgery, treatment focused on calming down the irritated nerve and restoring mobility. Interventions included: <\/span><\/div>\n<ul>\n<li><span>Neural flossing<\/span><span>\u202f(gentle nerve mobility drills) <\/span><\/li>\n<li><span>Soft tissue mobilization<\/span><span>\u202fof the calf and surrounding tissues <\/span><\/li>\n<li><span>Dry needling<\/span><span>\u202fof the gastrocnemius, soleus, and fibularis muscles <\/span><\/li>\n<li><span>A simple\u202f<\/span><span>home program<\/span><span>\u202fof nerve glides and foam rolling <\/span><\/li>\n<\/ul>\n<h2><strong>The Outcome: Back in the Saddle <\/strong><\/h2>\n<div><span>Within just three visits, the rider noticed a major difference: <\/span><\/div>\n<ul>\n<li><span>His numbness and tingling were gone. <\/span><\/li>\n<li><span>Pain dropped to almost nothing (1\/10 at most). <\/span><\/li>\n<li><span>He was back to long-distance riding, without worrying about surgery. <\/span><\/li>\n<\/ul>\n<div><span>By focusing on a thorough eval, patient history, and clinical findings rather than the dramatic findings on imaging, conservative care helped him fully return to the activities he loves. <\/span><\/div>\n<h2><strong>Why This Matters<\/strong><span> <\/span><\/h2>\n<div><span>This case is a reminder that\u202f<\/span><span>imaging findings often do not correlate with symptoms<\/span><span>. Pars defects, disc degeneration, and narrowing are common, even in people without symptoms, especially as we age.
