CMS had posted the most recent version of the OASIS C manual for users. Title is “2011 OASIS C Manual”.
Also posted is the file “OASIS C Guidance Manual Errata 12-11”.

Hi Eileen,
I have been trying to research info regarding co-signing PTA notes in California. Our CA regs have changed and we no longer have a co-signing requirement/weekly case conference requirement.
But I am wondering what Medicare requires for home health. I believe outpatient (part B)requires co-signing, but I haven’t found anything about home health.
Do you have any references for me??

Hi Julie,
I have never seen any Medicare home health (part A) requirements regarding PT co-signing PTA notes. Since 2011, visits are coded by a G code to show a visit provided by a PTA vs PT and re-assessments by a therapist are required at key time points. As long as the PT and PTA team are complying with those requirements ( and the plan of care, conditions of participation standards), the Medicare requirements would be met.

What type of caregivers are included in m2100? Are we to include private pay, waiver and passport aides?
Jan, the CMS guidance manual indicates that the caregivers to include here are any caregivers EXCLUDING home health agency staff.
I am not familiar with passport aides (this is not available in my area -NYC) but if they are not from the HHagency, then I would count them as a caregiver.

Hi Cathy, your scenario is possible as CMS has not given a rigid interpretation of the 13 and 19 re-assessment visits when there are multiple therapies actively providing care just emphasizing the re-assessments are completed by the 13th or 19th visit.
When only 1 therapist is actively visiting, then the re-assessment must be made on the 13th or 19th visit.

A staffing company just told me that when only 2 therapy(PT and ST) where involved in a case. A reeval on the 10th visit by ST and 13th for PT was within the range for the reevals. Is this correct?
